I've seen my boss do this a few times on the forklift, I never have. I don't understand why you wouldn't just let it hit something...it would probably stall if you left it. This was a mixture of stupidity, inexperience and the guy was just unlucky.

i actually saved a kids life once, he pulled up parallel to a 40yard dumpster and he climbed up in between the safety cage and the lip of the container, (why ill never know) anyway, his knee hit the shifter with the wheels turned, and it pinned him in the chest, without even thinking i jumped on the forklift threw it in reverse and watched him fall to the ground wheezing, he had to go to the hospital but he turned out ok. you shouldve seen the bruise!! it was huge!

I've been driving forklift for over 15 years and this is one of the best instructional videos that has dropped the ball. I guess we aren't used to building roads like that on this side. Probably the overall temporary nature of the construction makes sense. But even then if a temporary method is used then full reclamation back to original contours should take place and I have seen a lot of forestry road that didn't even meet that minimum when deactivated, I am probably biased when I see the rules I have to abide by and those which forestry allows for wood cutting conglomerates. Thanks for the info, I learned a bit.

It looks to me like poor water managment as well as poor puncheon placement. As you can see on the map it looks to me like a culvert was to be placed in the area of work wich would be the cause of water saturation of the soil. As for woofybigj commment. A properly punched and ballasted road is very stable as long as the water has been managed. To full width strip and endhaul a heavily spoil area is insane if you can overland.

Using puncheon for road building is called overlanding, you have to do so because the spoil is to deep and if you were to do a full width strip you would have no where for the water to go when you were done, besides the huge costs for end hauling the material away would be uneconomical. As well there is nothing wrong with puncheon roads as long as its done correctly and enough ballast is used to hold up the logging trucks. Poor water managment and poor puncheon placement here in my opinion.

Has anyone asked why BC forestry allows the use of "Puncheon" in road building. Anyone else, myself included involved in the oilfield on the otherside of the mountains knows that burying wood is the biggest no no you can do. (Unless its muskeg then its called corduroy). So "WCB" how about making them remove the wood,strip the soil properly and key in the fills, that way the hoe is always on a solid and stable surface

I believe that the structure on the cab was only a (FOPS)Falling object protective structure, WCB dosent require a (ROPS)roll over protective structure on a track hoe. I have been operating excavators and cats for the past 15 years in road construction and none of the exavators have a ROPS, just a FOPS required to work in the bush.

Nah, very few hoes have rollover protection (ROPS). Looks about like an EX200, something that Hitachi nor any other manufacturer of a machine that size would expect to be rolled. Operator must've been lucky, that cab got pancaked.

Feel for you. This past summer I almost lost my last 3 fingers on my right hand the first day on the job. The newspaper production plant skipped my safety training, had me doing a job i wasn't supposed to do, and had the belts unguarded (OSHA required guards. Luckily I only suffered 2 and 3rd degree friction burns, but big business clearly is a careless machine that exploits workers.

I cant imagine the pain this girl must feel, but I feel incredibly lucky from only suffering severe burns and scratches on my hand when it got stuck in an airport conveyor when I was a child (They needed someone with small hands to pull out my cousins tickets which were stuck in the conveyor belt, the worker didnt turn off the machine properly.)

A lady I worked with told me she got the tip of her finger cut off by a saw. Now she has a phantom finger. She said that even though the end of her finger is missing, sometimes the nerves make it feel like her whole finger is still there. Man I almost did a backflip when she told me that! Crazy but true.

terrible i feel for you it shouldn't have happened. there are so many employers that don't train enough. i had my thumb cut off almost the same way. the pain went on for months and the cold weather makes it very sensitive. but don't let it hold you back. i guess in a way it could have been a lot worse.

When I was five, there were these roller thingies at the pizza shop my grandma was manager at. and they stuck dough in there and it came out in a thin layer the other side, i stuck my left hand in. It crushed my middle and my index finger. It didn't do much harm, but my middle finger is very flat and my index is flatter than my right's. I lost the end of one of my fingers in a transmission gear on a metal cutting lathe. It didnt hurt at first but after awile the pain is unreal. The Dr. said it would hurt more because it was crushed off rather then cut off. Ya it was a stupid thing to get to close to. but we all do stupid things sometimes. The worst thing you can do is tell someone (well that was a stupid thing to do) I wanted to smash anyone who said that to me. One guy did and a year later he grabed a moving V belt. Smart hu?

Just awful, feel so bad for that kid and his family. I am glad I made it through all my "learning experiences" ok. It really is up to all of us to throw in our 2 cents and mentor and train these new workers properly. Learning to do by failure first, just isn't and option, do it right or don't even start the work.

What a terrible way to die! I've operated chippers before, and here's what I have to say: ALWAYS, ALWAYS have 2 or more people with you on the ground near the chipper when operating it. They could have stopped the thing in an emergency like that.

I was in a tree once when the ground man threw my rope in the chipper by accident. Luckily the foreman was watching and jerked my rope out just in time. What a frantic experience it was watching my rope unwind into the chipper.

I wonder if the worker was an immigrant. They have a higher rate of accidents due to lack of knowledge about the equipment they work with. These jobs such as a ground man is entry level and take no experience. Tree climbing takes skill and practice. Poor guy died after a few days on the job. I bet the foreman didn't train him well.

Thank God for brains!! The first chippers I worked with had no feed drums. It taught us respect for the chipper. With no feed drums, you have no time to stop the feed. At least the drums let you hit the safety bar. Now, you have to be a MORON to go through a chipper.

such a nasty way to go! please who ever you are be safe and be seen. we all see kids using stuff in the work place that they shouldnt be using i.e. fork lifts, i nearly got impaled by a young kid on a forklift that thought he was driving a go cart! yes i gave him a big slap! i drive for a living to keep my family, newbys shud be partnerd up with someone who they can learn from, not just thrown into a job and expected to do it like a professional. health and safety first!!!

Chippers are a great tool, but a dangerous one too. I note the guy had only been on the job " a few days".... You would not let a person drive a car without a license or training, or use a chainsaw without PROPER training.. likewise, this accident COULD have been prevented if the operator had only 'played by the rules'

Ive got a 12" vermeer drum chipper. The sales rep told me a story about a 14yr old boy who went thru a chipper. He was working with his dad on his farm. The kid stood on the feed tray as his old man drove to the next tree. He slipped, engaged the feed roller and went thru feet first.They didnt find much of him.

most accidents occur with this type of chipper vs. the old "whisper" chipper. you would think otherwise, but a whisper chipper will earn an operators respect the first time it snatches a glove off. its so loud too. the roller type is more quiet and draws brush in slower. you should always wear leather gloves not "gauntlet" or cloth gloves.

i think that the feed table should be raised at an angle and the safety bars moved a little out ward that way the only way a person could be dragged in is if they were lifted off the ground and dropped in but with the safety bar further out making it easyier to stop the engines

i do this kind of work for a living i work with a bandit chipper its a pretty big one it will take whole trees up to 18 inches round these machines deserve ur attention and respect if u dont do this then ur as good as dead.this is a very unfortunate accident to happen to this bloke i couldnt imagine what would have been going through his mind while this was happening to him he must have been so scared.my heart goes out to this mans family and friends.

chippers are excellent machines when operated properly, however they shouldn't be operated if the safety devices are non functional. A lot of care should be taken when operated and the operator needs to use common sense and be familiar with all standart operating procedures. Always load from the side and make sure there is a second person nearby in case of emergency

the driver was obviously trying to pull it out, hence the dirt! he may have left the tractor in gear, or it is possible for the drive train to jump back into gear. I had an old car that would jump back into gear? RIP brother : )

mabey three years ago i was at a big halloween party at a big farm at my cosins grand parent farm and at night a 9 year old kid fell out of a hay tractor thing and he got his head ran over and he got scallped some people think his 17 year old brother pushed him out and i was on the tractor when it happened along with about ten other people and the tracktor rose in the air and people were thinking it was a rock and the driver went along with the tractor and the 17 year old and my sister saw it
