That was 100% operator error, it has nothing to do with propper training. Common sense would tell you not to operate a piece of equipment like that. Would anyone stand outside of their car and put it in gear? NO! And if he didnt know how to operate it, he should of refused untill he got training, instead of trying to impress the boss. You as an employee have the right to say no without training.

at a full lock on the wheels its possible to go in circles, however when the wheels are fully locked it takes more power to turn it therefor causing the forklift to move even slower, i dont believe his version of events, i also have never and would never pull such a stunt and would severly repremand anyone i seen doing it

horseplay is a big no no at frito lay if they even catch you bumpimg some one you can be terminated we dont drive these lifts we got crown stand ups but im sure if we had those some people would try to do that. only bad thing i heard was before i was there a lady had her feet under the lift and the lift driver was rocking back and forth(he didnt know her feet were under the lift) and she lost her toes.

I had a "foreman" put a forklift in reverse like that with his foot under the front wheel. He was 28, well trained, and not tired. I didn't crush his foot but he was hurting for a long time.

2,000 pounds? Just for a reference, the forklift in the video (I believe is a Kalmar P50BX) weighs about 8,155 lbs. I'm a forklift mechanic and have drove a forklift at almost every job I have ever had. If you get into this situation, its our own fault, unless you haven't been properly trained.

A fork lift goes very slow without the pedal pressed (you could crawl faster than it's going) and a two thousand pound forklift just doesn't go in circles unless you are turning the wheel yourself....sounds like somebody was fooling around.

I've been driving forklift for over 15 years and this is one of the best instructional videos that has dropped the ball. When I was five, there were these roller thingies at the pizza shop my grandma was manager at. and they stuck dough in there and it came out in a thin layer the other side, i stuck my left hand in. I lost the end of one of my fingers in a transmission gear on a metal cutting lathe. It didnt hurt at first but after awile the pain is unreal. Has anyone asked why BC forestry allows the use of "Puncheon" in road building. Anyone else, myself included involved in the oilfield on the otherside of the mountains knows that burying wood is the biggest no no you can do. (Unless its muskeg then its called corduroy). So "WCB" how about making them remove the wood,strip the soil properly and key in the fills, that way the hoe is always on a solid and stable surface

I believe that the structure on the cab was only a (FOPS)Falling object protective structure, WCB dosent require a (ROPS)roll over protective structure on a track hoe. I have been operating excavators and cats for the past 15 years in road construction and none of the exavators have a ROPS, just a FOPS required to work in the bush.

such a nasty way to go! please who ever you are be safe and be seen. we all see kids using stuff in the work place that they shouldnt be using i.e. fork lifts, i nearly got impaled by a young kid on a forklift that thought he was driving a go cart! yes i gave him a big slap! i drive for a living to keep my family, newbys shud be partnerd up with someone who they can learn from, not just thrown into a job and expected to do it like a professional. health and safety first!!!

Chippers are a great tool, but a dangerous one too. I note the guy had only been on the job " a few days".... You would not let a person drive a car without a license or training, or use a chainsaw without PROPER training.. likewise, this accident COULD have been prevented if the operator had only 'played by the rules'

Ive got a 12" vermeer drum chipper. The sales rep told me a story about a 14yr old boy who went thru a chipper. He was working with his dad on his farm. The kid stood on the feed tray as his old man drove to the next tree. He slipped, engaged the feed roller and went thru feet first.They didnt find much of him.

most accidents occur with this type of chipper vs. the old "whisper" chipper. you would think otherwise, but a whisper chipper will earn an operators respect the first time it snatches a glove off. its so loud too. the roller type is more quiet and draws brush in slower. you should always wear leather gloves not "gauntlet" or cloth gloves.

i do this kind of work for a living i work with a bandit chipper its a pretty big one it will take whole trees up to 18 inches round these machines deserve ur attention and respect if u dont do this then ur as good as dead.this is a very unfortunate accident to happen to this bloke i couldnt imagine what would have been going through his mind while this was happening to him he must have been so scared.my heart goes out to this mans family and friends.

chippers are excellent machines when operated properly, however they shouldn't be operated if the safety devices are non functional. A lot of care should be taken when operated and the operator needs to use common sense and be familiar with all standart operating procedures. Always load from the side and make sure there is a second person nearby in case of emergency

18 yr of doing this job and I have been hurt by chippers more than once. There are so many reasons why someone could have been killed by a chipper. Inexperience is the biggest factor but time on the job will kill you fast. Old timers who say I've done this 1000 times before will be dead as fast as the the new guy.

The same thing happened in Perth, Western Australia. A 16 yr old nephew of the chipper owner was helping him on school break. The owner plus another adult employee were with him. The adults turned their backs on the chipper, to pick up vegetation, while the 16 yr old fed some limbs into the chipper. The adult workers turned back to the chipper to see the last of the 16 yr old disappearing into the chipper. Apparently he kicked at or grabbed a limb, got snagged on it, and got dragged straight in.

Similar happening here in Wisconsin. Only the crew watched the guy go all the way through, as none of the emergency stop or reverse controls worked. And he owned the business, so lack of experience was not to blame. Apparently there was a jam, and he went in to kick it with his foot, got caught by the machine and pulled all the way through. Don't know who I feel sorrier for, his family or the guys who had to see that.

I heard about this somewhere, only I think it was a different accident. One guy was standing by the chipper feeding in limbs, and one tree limb had a branch that was sticking out.The branch caught the man and he got pulled into the chipper....

Your explanation, about the operator being afraid to go down with the tractor, makes a lot of sense to me. When putting it in that perspective, the act of jumping out while tractor is still in gear, isnt so stupid after all. If the tractor would have slid into the pond, he would have never been catched by the tires while they were slipping backwards...

I work in a warehouse and our forklifts will not start without the seatbelt fastened and the engine will shut off if the seatbelt it unfastened, we haven't had this forever though and about 15 years ago we had someone who was crushed to death in a similar situation.

Some of those Forklifts out there dont seem to have enough Counter-Balence for those heavy lifting jobs some of them do. You would think that they would becouse they are used to pick up big things. I used to work at a Warehouse. One of their forklifts was a short little bugger. It was being used by a employee who lifting a truck bed with my friend close to it. The fork lift tipped forward and my friend was cought under it and got baddly injured.

I drive this exact machine and they are nice forklifts the problem was the ramps and probably rushing due to employer production schedule and yes lighting. If you roll or tip hug the steering wheel with all your might brace your feet under the dash and roll with it it will hurt but if you try to jump this is what happens. and it happens fast.

That's not so dumb as it seems. In my country it is obliged by law to use safety belts or safety doors -they do exist- in forklifts. You get used to them and you will be thankfull for having them, if your forklift tips over.
